Use of flowchart to mention challenges is good, you can add some lack of infrastructure related points.
Next part is important and is the crux of the answer, so you need to mention each scheme with its targets and objectives. That will explain how they seek to solve the problems of urbanization.
PM Awas Yojana and DAY- NULM are some major schemes that are missing, also explanation for AMRUT needs to be better- from what you have written it is not clear what the scheme is about. Conclusion is fine.

After that, you can mention that inspite of these schemes, multiple challenges still remain and then challenges can be mentioned.
